Healthcare Quality Improvement & Risk Management

Perhaps the most widely used improvement model is the PDSA model which is what you will use for your self-improvement project in this course.
You have the autonomy to choose your project from this list:
accepting criticism
budgeting (personal or family)
communication skills
healthy Lifestyle (includes weight loss, diet and exercise)
smoking cessation
stress reduction
stress related to time management
time management.
Due to time constraints and the scope of this course, your measures and resulting data display has been done for you.
Use this form for your work:  Just click in the box and type, although with extensive dialogue, the format might change, but it will not affect the content of the document.

What is it you want to improve: (Also referred to as your Aim Statement)
Smoking cessation.

Full smoking cessation
By tKLHhe end of the year.
How much: By when:

Plan
By completing the above, you have described how you will know if the changes create
an improvement.

I will use a 7 weeks likert survey and a line graph.
What Likert Survey and run chart will you use?

Complete to here for LP3.2 Drop Box: Project Topic

This is where you continue your planning stage (perhaps the most important phase of the PDSA cycle). Find evidence-based information on best practice related to your topic. Remember, in quality improvement it is not always up to you to reinvent the wheel. What works for others and supported by studies and research should work for you. (However, always give them credit when credit is due). Although you will be using the Likert survey, you are required to provide the “theory” behind you work.
In your update for Week 4, provide a review of how you have implemented the Plan stage of the PDSA Improvement Model (250-350 words). Copy any paste your response to the LP4.2 drop box.
The first step of quiting smoking is having a reason why one really wants to quit .There are a number of reasons of wanting to quit smoking which include:
Improving health-this is mostly the main reason. In some cases, quiting smoking because of health issues comes from the doctor’s advice but most times it is a self madedecision.oking increases once risk of throat cancer, lungcancer, heartdisease, emphysemagumdisease, ulcers and high blood pressure. In some instances the victim just wants to feel healthier by avoiding smoke caused coughing, sore throats and poor athletic ability thus the urge to quit.
Better looks-smoking can result to stained teeth, dull skin and wrinkles therefore an urge to quit.

Plan

In the ideal situation, you would begin the DO stage by testing on a small scale.
Here you will carry out test changes based on your evidence-based research and the Likert Survey (which is based on evidence-based resources) list them here:
Did you start the day without a cigarette?
What are you gaining from quiting smoking?
Are you keeping your hands busy?
Have you being carrying a lighter, matches or cigarette?
Have you kept away from smokers?
Hve you been  getting support from others for quiting?
Hve you been exercising?

DO

My aim is to be able to totally quit smoking in 7 weeks.I have stopped starting my days with a cigarette, kept away from smokers, stopped carrying cigarettes, kept myself busy and done a lot of exercises.I have chosen these changes because they are helping me to avoid smoking. They are evidence based.Ihave been recording data on my progress incourse of the 7 weeks and I intend to display it in  a table and a line graph.I have incorporated the PDSA by planning on how I will  quit smoking cigarettes ,and and doing everything necessary to help me quit smoking.

Discuss the progress of the Do Stage your improvement project, include:
1. Your aim statement.
2. Describe the change you have made, is there more than one?
3. Why you chose these changes, are they evidence based?
4. What data are you collecting and how do you intend to display it?
5. Provide a brief summary of how you have incorporated the PDSA Improvement Model up to this point.
Length (at least 250-350 words).

Copy and paste the above into the LP5.2 Drop Box

Apply Risk Management thinking to your self-improvement project. Answer:
what can potentially go wrong
what are limiting factors for success

1.I can go back to smoking cigarettes.
2.The limiting factors are staying close to people who are smoking or taking caffeine because it will increase my urge of smoking.

3.To prevent failure am awarding myself for every positive step, getting encouragement from family and friends and avoiding smokers.

what can be done to prevent failure?

Study
Copy and paste the above into the LP6.2 Drop Box
Use the space below to document problems and unexpected observations:
You will fill out the Likert survey at the end of each week and fill in the box below with problems and/or unexpected observations as you go along.
Week
Observation

1

Decreased blood pressure
2

Improved sense of smell

3

Improved sense of  taste

4

Exercise tolerance increased

5

Increased energy level

6

Minimal urge to smoke.

7

Coughing and fatigue reduced

Continue to utilize your worksheet, however there will be no need to submit any part of it in the drop box for here on out.
Although listed in steps, you begin the study stage right after implementing your first change by analyzing the results of your actions.
Again, in the ideal situation, the team would analyze the results and compare to your predictions.
Project Title
By filling out your Likert Survey each week, you are able to study the results of your efforts with the run chart that auto-fills with your survey results. Run charts provide a data visualization at a swift glance and is one of the most useful data displays.
Example of a well-prepared run chart:
Likert Survey score
Project Weeks

When you fill out the Likert survey provided to you, the run chart will be auto-filled with your results.
In the study phase you need to compare the actual results with your expectations – are you headed the right direction?
Predicted
Actual
Less craving of ciggarets
Better sense of taste

Improved health
Saving money

Yes.I have decreased the urge to smoke
Yes.My sense of smell and taste has really improved.
Yes.I have reduced coughing.
I have been saving more cash.

After  implementing the changes ,positive progress was experienced.
Summarize lessons learned during and after implementation of the changes.

Submit to LP 5 dropbox
In the ideal situation the team learns from its successes and failures to make a new plan in the PDSA cycle.

Act

See page 126 in your Spath textbook for a comprehensive PDSA Improvement Project report.
Remember, the Act stage is both the final stage of your process and the first stage of your next cycle.
If this were a small test, you would then implement changes on a larger scale (yes, another cycle) and track performance and data over time (which you have been doing for seven weeks).
A significant piece of the Act stage is to Plan how you will sustain or your efforts.
How will you prevent reverting to old ways of doing things?
Have you stabilized the process?

Yes I have stabilized the process and totally quit smoking.
Do you practice your changes every day, sometimes, never? (Can you see the entire PDSA cycle starting all over again?)
I practice my changes every day. There is no need to start the PDSA cycle all over again.
Apply Reliability Management Skills to sustain your efforts:

Dealing with Risk
Yes
No
1.
Is this project valuable to you?

HA3110D – Quality Improvement and Risk Management
LP10.1 Assignment: Organizational Quality Statement
For this assignment, consider the organization you now work in, or if you are not currently employed, consider your last employer.
Directions
Compare the culture of your current employer (or last employer, if not currently employed) to Demings’s Quality Principles:
.Help people do a better job.
.Drive out fear.
.Break down barriers.
.Restore pride of workmanship.
.Make quality everyone’s job.
Does this employer lead by example? Encourage open dialogue? Does management seek views and ideas? Does everyone have clear expectations for their expected level of performance? Do leaders provide reward and recognition for a job well done?
With that in mind, answer these questions:
1.If there was no bonus at stake, or if everyone got the same bonus for achieving collective goals, would your work deteriorate? Why or why not?
2.If other people on your team were to get 100 percent of the bonus but you didn’t, would you feel motivated or de-motivated? Inspired to excel or depressed? Rewarded or punished? Be specific. Explain thoroughly.
