She finds little joy in her life but cannot pinpoint any particular concern or event causing this problem. The PCP asks Angela to fill out a Beck’s Depression Scale, which indicates she has moderate depression.

Evaluation summary

 

Intended use

The intended use of this evaluation summary is to assist in evaluating the effectiveness of the evidence-based intervention in relation to achieving the intended goals and objectives. Goals are long-term targets that an individual or organizations aims at achieving in the long run (Hutahean, 2019). Objectives on the other hand, refers to the specific result that a system or a person aims to achieve within a short time frame and are more specific and easier to attain (Lau, 2016). In achieving this, the strategic plans will be assessed whether they serve the goal that was intended by applying the strategy. Among the intended objectives are making hospitals infection-free and reducing challenges faced by healthcare sanitation officers in the process of infection control. Also, adoption of administrative measures that reduce contamination sources and use of enough workforce in the infection control team. Through achievement of these objectives, there will be better sanitation management and reduction in the increased rate of hospital-acquired infections which are the goals of the intervention.

Program description

The program purposed to apply the strategic plans so as to achieve the objectives and at the long run achieve the goal of reducing the infection and better sanitation management. The strategic measures proposed are viable in that they have been formulated based on evidence collected on the causes of infections and other failures of the system (Bryant, 2016). For instance, cross-contamination in hospitals has been attributed to the poor cleaning of the contaminated devices. In achieving an objective such as making hospitals infection-free, the plan is to use technologies such as antimicrobial markers, hand hygiene monitoring technology and room cleaning records (Van, 2018).

Evaluation focus

The strategic measures will be marched to the desired objective to identify if it was a success. For instance, application of the technologies such as hand hygiene monitoring technology will be matched if it was able to reduce infections through contaminations in hospital facilities. Also, the strategic measure on disinfection and cleaning through making water and detergents as a strategic measure will be matched against the objective of infection control and prevention program. In addition, evaluating the effectiveness of the strategic plan on buying equipment to reduce the risk of infection and disease spread will be assessed by whether there will be reduction in infection according to the data provided by the doctors.

Methods

This will involve evaluation indicators as well as performance measures which will require data from patient registers indicating the number of infections reported and interview reports from the patients. Also, questionnaires filled by sanitation staff will be used in determining the impact of the intervention. These methods will serve the role of collecting both qualitative data and quantitative data such as the number of infections reported (Wang, 2018).

Analysis, interpretation and use of the plan

The collected data will be analyzed and interpretation made so as to draw appropriate conclusions. The interim results will be shared with the management of University Plaza Nursing and Rehabilitation Center. A correlation analysis will be drawn to show the relationship between every measure and the desired results. The results will be used to determine if the plan proposal is effective or whether amendments should be made.
