Dudley (2009) points out that social work practice is usually embedded in programs. While you looked at practice evaluation using single-subject design in Week 3, this week, you shift focus to program evaluation. Program evaluation serves many purposes, including accountability to funders and to the public. Often, funding sources such as government agencies or private foundations requires periodic program evaluations. These evaluations can help provide answers to many different questions, and can contribute to improvement of services. There are a variety of program evaluation models that are appropriate for addressing different questions as well as facilitating the collection and analysis of many different types of data.

To prepare for this Discussion, identify a program within an agency with which you are familiar, which could benefit from process evaluation and outcome evaluation. You do not need to identify the agency in your post. Also, review the different evaluation models highlighted in this week’s resources (needs assessment, program monitoring, client satisfaction study, outcome evaluation, or cost benefit study).

By Day 3

Post a brief summary of the program that you selected. Recommend a program evaluation model that would answer a question relevant to the program. 1. Summarize the episode you watched.

On the night of February 19, 1998, Sarah Danlin was in a bar celebrating Mardi Gras with friends. A man began flirting with her eventually luring her away from her friends to an empty room where he and a friend raped her. When Sarah reported the crime to detectives LaMontagne Sr. and J.R. Smith, the detectives disagreed on the credibility of the victim’s statement. Shill, S (2007) Smith believed that the young Sarah was being deceptive due to the fact that it was Mardi Gras and she has many beads around her neck. Smith even said, “she was asking for trouble”. Smith was protecting a rapist because he knew one of the victim and was led to believe he was a “good kid”.

Following the rape, Sarah dropped out of medical school and became obsessed with Jack the Ripper, a famous serial killer. Targeting men, she would lure them away from the crowds by seducing them with promises of sex. Once alone with the victim, the suspect would slash their throats sometimes eviscerating them as well as taking items from her victims. Sarah also sent letters to the detectives with the heading, “Yours Truly).  “ Shill, S (2007) Sarah was believed to be organized, calculating, and had maybe previously stalked the victim before the murder. She may have identified with Jack the Ripper because she believed she had lost her own identity issues either by one traumatic event or over the course of her childhood life. Sarah Danlin became a misandrist serial killer because of the rape that was ignored and destroyed her life.

2. Outline the personality disorders that were present.

Personality is the way of thinking, feeling and behaving that makes a person different from other people. An individual’s personality is influenced by experiences, environment (surroundings, life situations) and inherited characteristics. A person’s personality typically stays the same over time. A personality disorder is a way of thinking, feeling and behaving that deviates from the expectations of the culture, causes distress or problems functioning, and lasts over time.1 The pattern of experience and behavior begins by late adolescence or early adulthood and causes distress or problems in functioning. Without treatment, personality disorders can be long-lasting. The personality disorders present in Sarah Danlin is Narcissistic Personality Disorder. Sarah believes she is doing a service by punishing these men with murder because of their sexual aggression.

  1. Describe the symptoms of the        personality disorders.

Narcissistic Personality Disorder as described in the DSM-5 as a mental condition in which people have an inflated sense of their own importance, a deep need for excessive attention and admiration, troubled relationships, and a lack of empathy for others. American Psychiatric Association. (2013).  Other Narcissistic qualities include, arrogant thinking, lack of empathy for others, manipulative and selfish.

Narcissistic personality disorder: a pattern of need for admiration and lack of empathy for others. A person with narcissistic personality disorder may have a grandiose sense of self-importance, a sense of entitlement, take advantage of others or lack empathy.

Sarah had no empathy during the murders, enacting how she felt about her own rape. After reporting her own rape and not getting any results Sarah took matters into her own hand and began vigilante killings. She believed she was ridding the world of other rapist so that women would not have to suffer.

4. Describe possible treatment interventions for the criminal displayed in the show.

The show didn’t really touch on treatment interventions with the criminal. She was arrested, the victim was taken for treatment, and the show was over.

5. Describe possible difficulties that may be faced during treatment.

There are two major types of antipsychotic  medication. Typical antipsychotics are older drugs, such as  chlorpromazine and haloperidol. Typical antipsychotics tend to be highly  effective, yet they often have severe side effects. Older clients are  more likely to have been prescribed typical antipsychotics before newer  atypical antipsychotics became available. However, as long as the client  is tolerating the medication, generally the treatment will continue.

Atypical  antipsychotics, or newer drugs, tend to be tolerated more readily, and  the side effects are less severe (Perry, Alexander, Liskow, &  DeVane, 2007). Since typical antipsychotics often have severe side  effects, they are used as a last resort when the atypical antipsychotics  are not effective. Nevertheless, both antipsychotic medications manage  the positive symptoms of schizophrenia or other psychotic disorders, including hallucinations and delusions.

For  this Discussion, review the media programs “Psychotic Disorder Case  Study: Harry” and“Psychotic Disorder Case Study: Ralph”and consider the  case study’s current antipsychotic drug treatments. Next, you will be  placed into one of two groups (i.e., Group A: “Harry” case study or  Group B: “Ralph” case study). You then will post to your specific group  about your assigned case study. You will then respond to a colleague  from the opposite group about their case study.

By Day 3

Post an alternative  antipsychotic drug treatment for your assigned case and explain why you  think it is more appropriate. Explain the potential negative side  effects and identify symptoms that may not be relieved by the treatment.  Then, explain one potential ethical concern related to this client’s  treatment and your role as a mental health professional once the client  has been stabilized on his medication.

Imagine that you have been seeing a client for a  year. Initially, treatment was tailored to address a diagnosis of major  depressive disorder. Over the course of your counseling sessions, your  client has begun to develop psychotic symptoms. Often,  counselors encounter ethical and clinical dilemmas in deciding the best  course of treatment for their clients. Considering the possibility that  a client might wish to avoid going on antipsychotic medication, could  you support this choice? Does the client’s awareness and understanding  of the voices mean no further action is required on the part of the  counselor? Since there appears to be no immediate danger to herself or  others, could the counselor disregard the symptoms?

As hallucinations and delusions can vary from those that  were seemingly borne of logic to the truly bizarre, this is an  opportunity to fully engage your most imaginative thoughts.

Leading a group of individuals who have suffered trauma can be difficult because the shared stories may result in further trauma to some of the members. Assessing the members and deciding how they will introduce themselves at the first meeting can be a difficult task. Helping these members begin the group therapy process is the first step in facilitating the group.

Post your evaluation of the group’s social worker’s leadership skills, using at least two items from each of the three categories found in the Toseland & Rivas (2017) piece (facilitation of group processes, data gathering and assessment, and action). Suggest another way the social worker might have initiated the group conversation.

Levy Family Episode 6 Program Transcript FEMALE SPEAKER: I know three of you did tours of duty in Iraq, and the others in Afghanistan. So I just wanted to follow up on that, talk about how you’re adjusting. [MUSIC PLAYING] MALE SPEAKER 1: You say adjust to, but there’s no adjustment. You’re just thrown back into your life like you’re supposed to pick up where you left off, but that’s a joke. Two years ago, I was dug in, pinned down by 50 Cal sniper fire, just praying the chopper would get me out alive. Now, the hardest part of my day is standing in the grocery store trying to decide if I want yellow or brown mustard  with my hot dogs.  JAKE: Nah, two six packs or a case.  FEMALE SPEAKER: You find that you drink more than you used to?  JAKE: Why not ask him if he finds he’s eating more hot dogs than he used to?  BILL: You know why?  JAKE: Why is that? Oh great, Buddha.  BILL: Because I’ve been where you are. You talk about booze like it’s some joke,  but nobody’s laughing. You can’t get adjusted to anything when you’re trying to  get loaded. JAKE: I guess you won’t be joining me for a drink at the bar later. I was going to  buy. FEMALE SPEAKER: No, that’s a good point, Bill. Sometimes we do things to  avoid dealing with unpleasant feelings, like adjusting to life back at home.  JAKE: What do you know about it? Give me a break. Back off, or I’ll make you.  BILL: I drink too much too. But I’ve had enough of you mouthing off.  JAKE: My wife’s had enough of me too. She’s the reason I’m here. We never used to fight. I never used to drink so much, but now I can’t stop myself from  doing either.  FEMALE SPEAKER: So why do you drink too much?  ©2013 Laureate Education, Inc. 1                  Levy Family Episode 6 BILL: It’s the only way I can shut it out, images of what I saw over there, horrible things that no one should ever have to see. I wake up some nights and I hear mortar rounds coming in. And I reach for my helmet and my weapon, but they’re not there. So I freak out. And then I see pretty curtains. TV’S on. And then I remember I’m at home. I realize I’m not going to get blown up after all. FEMALE SPEAKER: Thanks for sharing, Jake. You make a good point. It can seem a lot easier to self medicate rather than face the fears, the bad memories that we have. So what do you think?
