Madeline: Pain and Suffering

Madeline S. is a 66-year-old German woman admitted to a home care agency for care related to end-stage cardiac disease and renal failure.  She has complained of chronic generalized chest pain, frequent cramps in her legs, and worsening arthritis pain related to her immobility.  The home care nurse has been in contact with Madeline’s physician almost daily for the past week and her analgesics have been steadily increased with little pain relief but an increase in nausea, constipation and sedation.  The nurse feels frustrated as she observes Madeline declining rapidly with worsening depression, withdrawal and weeping.  Madeline’s neighbor has noticed that her lights are left on 24 hours a day and the nurse has noticed that Madeline has several rosaries and prayer books now at her bedside.  Madeline has refused referral to hospice but the home care nurse has requested a team conference with assistance from the local hospice affiliated with the home care agency.

Case Study: A 65-year-old woman was just been diagnosed with Stage 3 non-Hodgkin’s lymphoma. She was informed of this diagnosis in her primary care physician’s office. She leaves her physician’s office and goes home to review all of her tests and lab results with her family. She goes home and logs into her PHR. She is only able to pull up a portion of her test results. She calls her physician’s office with this concern. The office staff discussed that she had part of her lab work completed at a lab not connected to the organization, part was completed at the emergency room, and part was completed in the lab that is part of the doctor’s office organization.

The above scenario might be a scenario that you have commonly worked with in clinical practice. For many reasons, patients often receive healthcare from multiple organizations that might have different systems.

As you review this scenario, reflect and answer these questions for this discussion.

• What are the pros and cons of the situation in the case study?
• What safeguards are included in patient portals and PHRs to help patients and      healthcare professionals ensure safety?

Thesis

You provided a “working” thesis in your topic email. Now, discuss any alterations you’ve made, or may need to make, as the process moves forward. For example, have you found a way to narrow your thesis’ scope, or will it be revised in a more persuasive direction?

Audience

You indicated a potential audience for your report in the topic email. Indicate whether your audience has changed since then. If so, explain how, and if your research has played a role in that process.

Research

Describe any previous research you’ve done that might be utilized in your report, research you’ve done since you received topic approval, and research you’re planning to do (what are the “gaps” you need to fill through research to write a quality report?).

Drafting

Provide some indication of how much writing you’ve actually done on the report to this point. Also, be honest. There’s no penalty for being a procrastinator as long as the job gets done.

Your email will reflect consideration of the guidelines provided on pages 100-03. Pay special attention to the following:
