You are a nurse on a Telemetry Unit, and you have four patients today.
Read the following patient assignment and figure out your day. Use a task sheet that you currently use, Use the one provided or design one to help you. You can also write each task on a post it note (color coded) to help you figure out your day. (You do not need to turn in your filled-out task sheet, but still do it, as it will help you determine if your sheet is going to help you). Fill out the schedule and answer the questions.
Patient 1:
58-year-old male, full code. Admitted 2 days ago for CHF exacerbation. Presented with SOB, Edema in lower extremities. Uses 3L of O2 at home but has been on 5L of O2 here. Has medications due at 0900, 1400, 1600 and 1800. No CHF education has been provided yet. Would like to ambulate today, he is usually a 1x assist with FWW. VSS. During bedside report the patient appears calm, no signs of distress and no complaints of pain. Bed strip/alarm in place.
Patient 2:
66-year-old female, full code. Admitted 3 weeks ago for cellulitis. Presented with swelling and shiny skin bilat LE. Patient has had persistent diarrhea for the last week, is now on rule out CDIFF. Her hemoglobin has been steadily dropping now at 9.1, serial h/h have been ordered q6h. She has a PICC line so you will draw them next draw due at 0930. She has a coretrak running fibersource at 35 ml, goal is 60ml. Rate needs to be increased to goal at 1000. She is only oriented to self and place. Last night she kept pulling on her coretrak so she was put in bilateral wrist restraints. She has medications due at 0900, 1000, 1200, 1300, 1500,1600, and 1800. She has multiple pressure ulcers on her bilat heels, coccyx, stage 2. She has dressing changes due today. On q2h turns max assist. At bedside report you see she is on an atmosair bed (normal hospital bed), sleeping, unlabored breathing. Restraints applied appropriately. Bed strip/alarm in place. VSS.
Patient 3:
42 year old male, full code. New admit at change of shift for PNA. The night nurse tells you “they just came up, all I did was tuck them in”. In report you get that he is a frequent flyer and usually here for ETOH withdrawal. CIWA protocol is ordered (for this activity he will need CIWA q4h). He also has medications ordered at 0900 and 1400. During bedside report you see that he is slightly irritable. He has no bed alarm/strip in place. He is a SBA. He has continuous fluids and IV ABX ordered but no IV pump/pole is in the room. VSS.
Patient 4:
82-year-old female, full code. Admitted 4 days ago for atrial fibrillation RVR. She has been receiving Coumadin and when looking at her labs you see her INR is 6.0. NOC notified MD and was given ordered to hold Coumadin. The monitor room calls you during report and says her heart rate is sustaining in the 140’s no prn medications are ordered for heart rate. She has medications due at 0900, 1200, 1400, 1800. She has difficulty swallowing, so she needs her medications crushed and put in applesauce. On bedside report she complains of pain in her back but otherwise feels okay. Unlabored breathing. No bed alarm/strip in place. 2x assist with FWW.
