Hi, I’m Dr. Rose Stein and I’m a clinical psychologist. I have been asked to assess a client’s mental health in the context of some challenging circumstances – Ben has a terminal illness and has been given 3-4 months to live. I could use your help in figuring out whether his depression is impacting his wish to die by physician-assisted suicide. What are the important factors to consider in Ben’s case? How do your own beliefs affect what you think about Ben’s case? Those are some of the questions you will answer when you Make A Decision. To answer those questions you will Investigate the Evidence I have collated for you. But before you Investigate the Evidence, take a minute to Consult the Research I’ve collected.
Consult the Research
You will be able to come here to Consult the Research anytime you wish. The Right to Die movement is a controversial one. Take a moment to Consult the Research, including looking at the evidence of the effect of depression on decision-making. Then you will Investigate the Evidence I have collected, before you Make A Decision to decide whether Ben’s decision making is being affected by his depression and what other factors to consider in his case.
The right to die movement
· In the USA, Oregon, Washington and Vermont have laws that allow terminally ill adults to get prescriptions for lethal doses of medications from their physicians. Patients must be capable of making health care decisions and have a terminal disease that will lead to death within six months (Public Health Oregon, 1997).
· As of early 2015, assisted suicide was also permitted in The Netherlands, Belgium, Luxembourg and Columbia. Although assisted suicide is illegal in Switzerland, it is only penalized if it is carried out “from selfish motives.”
· In November 2014 Pope Francis denounced the right to die movement, saying it is a “false sense of compassion” to consider euthanasia as an act of dignity, when in fact it’s a sin against God and creation.
Living with a terminal illness
· Studies of people with serious illnesses have revealed that being a burden on family is a significant concern about the end of life (Singer et al 1999; Steinhauser et al 2000). More recently, Chochinov et al. (2005) studied patients nearing death and found that the more of a burden a patient felt, the lower they reported their will to live.
· Research by Mount Sinai School of Medicine (2012) studied 3,209 Medicare recipients during their last five years of life, and examined their out-of-pocket health care expenditures. Those with cancer spent an average of $31,069, and 43 percent of people spent more than their total assets, (excluding the value of their home).
· Best estimates are that between 15% and 50% of cancer patients experience depressive symptoms, and 5% to 20% will meet diagnostic criteria for major depressive disorder (Chochinov et al 1994; Hotopf et al 2002; Massie et al 2004; Shuster et al 1999; Bottomley et al 1998; Keith et al 2007).
